Nowadays, most patients with congenital heart defects (CHD) survive into adulthood. Unfortunately, despite relevant residuals and sequelae, follow-up of adults with congenital heart defects (ACHD) is predominantly performed by nonspecialized physicians or hospitals with little experience in the field of ACHD. Major problems in the long-term course include CHD-related residua, heart failure, cardiac arrhythmia, valvular heart disease, pulmonary vascular disease/pulmonary hypertension, infective endocarditis and aortopathy, as well as cardiac and noncardiac comorbidities. Many problems in ACHD manifest differently from those of acquired heart disease, and treatment regimens from general cardiology cannot be directly applied to CHD. It should be noted that even simple postoperative cardiac defects, which until recently were considered harmless, can develop serious CHD-related complications with advancing age, a fact that was rarely anticipated. Therefore, the treatment of ACHD has many special features and requires special expertise. This position paper of the German Cardiac Society provides a highly up to date overview and highlights current data on epidemiology, heart failure, and cardiac arrhythmias in ACHD.
