Altersabhängige Veränderungen der axialen Haarfestigkeit in der Pathogenese des Pilonidalsinus

Abstract

Background: A pilonidal sinus is caused by the penetration of sharp hair fragments into the skin of the upper gluteal fold. The pilonidal sinus disease frequently occurs at the beginning of puberty; however, it is unknown whether this is associated with the firmness of hairs. Therefore, the axial firmness of hairs from the occipital region of the head and the contribution to the development of pilonidal sinus disease were investigated. Method: Hair samples were taken from a total of 416 persons between the ages of 5 months and 90 years, of which 340 were controls and 76 were patients with pilonidal sinus. Furthermore, details of the sex, age, body mass index (BMI) and smoking behavior were documented. Each hair was tested 6 times and the mean value of the 6 measurements was used for the analysis. Results: The axial firmness of the hairs of the occipital region of the head showed relevant age-related alterations and was highest in the second decade of life. The regression model showed a relevant contribution of the axial hair strength (odds ratio, OR, 1.68, 95% confidence interval, CI, 1.2–2.33; p = 0.002), BMI (OR 1.11, 95 % CI 1.06–1.18; p < 0.001) and a negative correlation to female sex (OR 0.15; 95 % CI 0.08–0.28; p < 0.001) for the development of pilonidal sinus. In contrast, nicotine abuse showed no effect (OR 1.06; 95 % CI 0.6–1.95; p = 0.78). Discussion: An age-related variability of the axial firmness of hairs could be shown, whereby the largest difference in the firmness of hairs between the controls and pilonidal sinus patients coincided with the age cohort with the highest incidence. In addition to the known risk factors, the axial firmness of hairs proved to be a relevant factor for the occurrence of pilonidal sinus disease.
