Abstract

Summary: Malignant melanoma is – despite increasing incidence in adults – a rare tumor in childhood. Nevertheless, melanoma should be kept in mind in case of a growing uncertain skin lesion even in children. Clinically as well as histopathologically, the diagnosis of malignant melanoma in childhood is challenging: the classic ABCDE rule is not applicable with the same acronyms in children. Additionally, classical melanoma subtypes only account for a small proportion of melanoma in childhood. Whereas a majority of melanoma remain unclassified or are atypical lesions whose dignity ultimately cannot be clearly determined. This histologic uncertainty must also be considered in sentinel lymph node biopsy. Especially in spitzoid melanomas, melanocytic cells are found during lymph node biopsy in up to 50 % without proven effect on the prognosis. Due to the low number of cases in children, further therapy should be analogous to the recommendations for adults after thorough interdisciplinary consideration. Any malignant disease in childhood represents an enormous burden for the patient, the family and the treating physicians, which is why a close interdisciplinary cooperation between the different departments is necessary. Due to this complex situation, the treatment of childhood melanoma should be performed at a designated center.
