Peripheral arterial occlusive disease (PAOD) is one of the most frequent vascular diseases with around 237 million affected persons worldwide. It shows an increasing prevalence, particularly due to risk factors, such as diabetes mellitus, hypertension, hypercholesterolemia and nicotine consumption. It is not only an independent disease but also an indicator for systemic cardiovascular morbidity and mortality, whereby the causes of death are mostly cardiac or cerebrovascular in origin. An excellent treatment requires an individualized initial planning considering conservative, endovascular and open surgical procedures. In the stage of intermittent claudication conservative treatment is initially given priority, especially structured walking training and consistent best medical treatment. Revascularization should first be carried out after these measures have been exhausted and considering the long-term stability. Aftercare is of key importance, whereby patient-reported outcomes should be considered in addition to clinical parameters. In cases of chronic critical limb-threatening ischemia (CLTI) a consensus rapid, individualized and interdisciplinary treatment is necessary. The choice between open and endovascular procedures should be carried out in a patient-specific manner, ideally within the framework of a structured vascular board. Modern procedures, such as microperfusion measurements can support the treatment planning and monitoring. For “no option” patients. innovative approaches are available to avoid amputation as far as possible. The quality of treatment is essentially influenced by experience and case numbers, whereby high-volume centers show better results. Uniform quality indicators are so far lacking. Overall, excellence in the treatment of PAOD is based on individualized planning, interdisciplinary decision making, structured aftercare and continuous quality assurance.
